Q. What is the CDPHP Crystal Run Program. Is this a new and unique option and if so what makes it different?
A. Yes, it is new and it is unique. CDPHP and Crystal Run Health Care have teamed up to provide quality care to members at a significant reduction in premium. With this program you must use Crystal Run affiliated providers in order for services to be covered which provides the framework for the increased savings.
Q. What about when I am traveling since there would not be Crystal Run providers available?
A.. You have coverage worldwide when you are traveling for emergency situations.
Q. That covers part of the provider question but what if I have a situation where Crystal Run for one reason or another is not able to provide a covered service that I may need?
A. These situations are handled on a referral basis meaning that, if necessary, you will be referred to a provider outside the Crystal Run Network and it will be covered.
Q. What plan options do I have?
A. There are three plan options including a High Deductible EPO, an EPO with cost sharing and an EPO with no deductible or coinsurance. Each plan can be purchased with or without a prescriptions which provides additional flexibility and cost savings for members.
Q. If I sign up for one of the plans, how long will my rate be in effect?
A. Rates are good for one year from the time you enroll. If April 1, 2011; rate good till April 1, 2012 etc.
Q. I have several employees, do they all need to take this plan?
A. No, as few as one person may choose the plan within your company.
Q. Will there be an administrative fee added to my premium
A. No, there will be no other fees added to your premium.
Q. Where do I get more information including benefit outlines, rates, enrollment materials and answers to any questions I may have.
